Aftercare Case Manager

Position Overview

As Aftercare Case Manager, you’ll work in one of our Emergency Domestic Violence shelters and provide outreach service support to women who previously resided in the emergency shelter system and are currently living in the community. This includes services and referrals such as assessments, counseling, service planning, arrears resolution, entitlements, medical, early childhood, educational, substance abuse, employment, and mental health services.

Your responsibilities will include:

  • Meeting with assigned clients on a regularly-scheduled basis in their homes in the community or in the office to provide supportive counseling and to review their progress towards remaining out of shelter.

  • Providing advocacy, information and referral services for housing, entitlements, employment, vocational training, and education.

  • Managing a high-volume caseload and maintaining electronic case records in AWARD’s and physical case records in compliance with regulations and policies set forth by HELP USA and the Human Resource Department of Homeless Services.

  • Working with awardees of the Domestic Violence Shelters’ annual scholarship to ensure that monetary awards are processed for employment, housing, education, and other valid purposes, and maintaining monthly contact with awardees to ensure continuation of permanent housing.


  • Bachelor’s degree required, with a degree in Social Work or a related field preferred.

  • Prior experience in case management, assessment, counseling, and crisis intervention preferred.

  • Computer literacy, particularly with Microsoft Office applications. 

  • Valid U.S. driver’s license required.
