Case Management Supervisor

Site/Program: Project Anchor
Location: Ronkonkoma, NY

What Youíll Do

If you want to help people at imminent risk of losing their homes, you could be a great Case Management Supervisor with HELP USA Project Anchor in Suffolk County and play a key role in delivery of a comprehensive, community-based homelessness prevention program.  Youíll report to the Assistant Program Director and guide a team of Case Managers and Housing Specialists who are providing emergency crisis intervention and housing stabilization services to individuals and families at risk of becoming homeless in order to prevent shelter entry (or re-entry).  

Your responsibilities will include:

  • Assisting your team in linking clients to immediate resources to maintain existing housing and providing direct intervention for problematic and/or urgent cases.

  • Ensuring that all team members are effective in meeting client needs by providing regularly scheduled supervision, holding regular team meetings and case conferences, facilitating training, and evaluating performance.

  • Identifying gaps in service delivery systems and assisting with developing solutions for program improvement.

  • Contributing to development of strategies, procedures, and service linkages geared toward meeting and exceeding contractual goals, objectives, and targets.

  • Ensuring accurate and timely documentation of all program activities as well as compliance with all program and company policies and regulations.

Youíre a great fit for this role if you:

  • Have a bachelorís degree in social work or a related field, and a Masterís Degree would be even better.

  • Have experience in case management, assessment, counseling, and crisis intervention.

  • Have experience providing client advocacy and mediation as well as strong verbal, writing, and negotiation skills.

  • Have strong organizational and documentation skills, with the ability to pay keen attention to detail, manage multiple tasks in a time-sensitive manner, and work in a fast-paced and demanding environment.

  • Are able to take initiative and be dependable and resourceful.

  • Are available to provide some evening and weekend coverage as needed.

It would be helpful (but not required) if you:

  • Supervisory experience in a case management setting.

  • Have a valid US driverís license
