Senior Case Manager

Site/Program: Mt. Vernon

Position Overview

As Senior Case Manager, you’ll report to the Team Leader and supervise a team of three Case Managers as well as manage your own case load.  You’ll provide assessment, counseling, and referral services to shelter clients; ensure that proper protocols are being followed; and perform quality assurance activities regarding delivery of services.

Specific responsibilities will include:

  • Supervising Case Managers and maintaining a caseload of 23 clients.

  • Following up on referrals to determine if they meet the criteria for admission to the shelter and verifying their appropriateness for referral to various programs.

  • Assisting the Team Leader in providing guidance and direction to Case Managers to ensure assessments (including psycho-social evaluations) are being performed, Independent Living Plans are being promptly followed, referrals to appropriate programs are being made, and integration of services is occurring.

  • Assisting the Team Leader in conducting chart reviews to ensure all required documents have been given to and acknowledged by clients and the delivery of services is properly coordinated.

  • Providing individual and group counseling sessions to assist clients in achieving independence, obtaining permanent housing, and the returning to their communities.

  • Ensuring that accurate and up to date clinical documents are being maintained and the required data is input into the AWARDS system by Case Managers in a timely fashion.

  • Attending and participating in weekly case conference meetings on the status of each client.

  • Coordinating the Money Management Program, exercising fiduciary responsibility on behalf of clients to ensure savings goals are being achieved and that clients are spending funds responsibly.

Requirements

  • Bachelor’s Degree in Social Work or a related field required, Master’s Degree in Social Work preferred.

  • Knowledgeable of relevant state and county regulations.

  • A minimum of two years’ experience in case management, assessment, counseling, and crisis intervention preferred.

  • Strong verbal, written, and negotiation skills, including experience in advocacy and mediation.

  • Strong organizational and documentation skills.

  • Ability to take the initiative, be dependable and resourceful, and pay keen attention to detail

  • Excellent organizational skills to enable successful management of multiple tasks in a timely manner in a fast paced and demanding environment

  • Knowledge of family systems approach to practice preferred.

  • Knowledge of team concepts preferred.

  • Computer literacy, particularly with Microsoft Office applications.

  • Able to provide evening and weekend coverage as needed.

  • Valid US Drivers’ license a plus.

Compensation:  $21.63 - 25.24/hr
