Aftercare Case Manger

Position Overview

As Aftercare Case Manager, youíll work in one of our womenís assessment shelters and provide outreach service support to women who have been diverted from the shelter system and are living in the community. This includes services and referrals such as assessments, counseling, service planning, arrears resolution, entitlements, medical, early childhood, educational, substance abuse, employment, and mental health services.

Your responsibilities will include:

  • Meeting with assigned clients on a regularly scheduled basis in their homes in the community or in the office to provide supportive counseling and to review their progress towards remaining out of shelter.

  • Providing advocacy, information and referral services for housing, entitlements, employment, vocational training, and education.

  • Managing a high-volume caseload and maintaining electronic case records in CARES and physical case records in compliance with regulations and policies set forth by HELP USA and the Department of Homeless Services.

Requirements

  • Bachelorís degree required, with a degree in Social Work or a related field preferred.

  • Prior experience in case management, assessment, counseling, and crisis intervention preferred.

  • Computer literacy, particularly with Microsoft Office applications.†

  • Valid U.S. driverís license required.
